The monument itself is situated outside the fenced area, near the road, dividing the cemetery in two parts. It is a gray granite square plaque with identical Ukrainian, English, and Hebrew inscriptions. A Magen David is carved at the top of the plaque. Several steps lead to the paved area with the plaque. Near the monument, there is a square column with plaques attached to it. They bear information about the Holocaust and the Jewish community in Ostrozhets. An information board with the site plan is also situated near the monument.
