The memorial is a light-colored stone slab rounded at the top, it is set on a black marble base rounded at the front.  On the back of the base, is a locked door that secures a small niche containing a scroll with the names of the Jews of Działoszyce, Skalbmierz, and Kazimierza Wielka, who perished in the Holocaust.
