The silver cylindrical coronet is divided into two parts: a base and an upper part. The convexed base is decorated with a band of crests encircling a flower. Between the crests there is a foliate pattern. The lower edges of the base are decorated with a pattern of a leaf framed by an arch and a pattern of a branch and leaves intermittently. The upper part is decorated with five smooth oval medallions each is framed by a band of a grooved pattern. A flower surrounded by two symmetrical scrolls is set between the medallions.

Each medallion bears an inscription in square linear letters which reads:

On the first medallion:

"Dedicated to the holy congregation Algazi may they witness the comfort of Zion"

Continues on the second medallion:

"From the woman Mrs. Mazal Tov she will be blessed above women of the tent. (see: Judg. 5:24)"

Continues on the third medallion:

"For the exaltation of the soul of her husband the important and praiseworthy David Gabai Dona may he rest in Eden"

Continues on the fourth medallion:

"Passed away at the day of 6 of Hashvan in the year 5673 [1913]"

"Nisan 5674 [1914] may his soul be bound up in the bond of life"

Between the medallions and the arches there are leaves. The upper and lower bands are decorated with grooves. The coronet has wavy edges decorated with the same crest pattern of the base. A bar with rectangular holes for the staves is attached across the lower part of the coronet.
