The following description was prepared by William Gross:
This amulet was printed in a Haggadah, but here is presented as a separate printed item of four pages. It is stated that the Haggadah is no longer available. Today it is found in many Haggadot and is a prayer/amulet written by Shimshon MeOstropoli that will provide protection the entire year after Pesach. The man who brought this prayer to press lists at the end of those of his family who perished at Aushwitz. This page was printed by the Neufeld family in Galanta, a Jewish publisher whose trade name was Kosmos and whose output is represented by five printed items in the Gross Family Collection.
