The leaf-shaped dedicatory plaque encloses a Hebrew inscription and is framed by a broad band, decorated with a scrolled acanthus stem following the plaque's form. The Hebrew inscription is arranged in fourteen lines and is written in square, outlined letters that read:
"אל/ שדי/ התכשיט/ והמפה הקדיש/ ה''ר (הרב) משה יוסף יצ''ו (ישמרהו צורו ויחיהו)/ ואשתו מרת אסתר/ מב''ת (מנשים באוהל תבורך: שופטים ה:כד)) על הנס שעשה/ להם השי''ת (השם יתברך) והציל/ אותם ואת ביתם מן/ הדליקה 'ברוך פודה/ ומציל' והיה בבואו אל/ הקדש (מבוסס על שמות כח:כט; ויקרא י:יח, טז:כג; יחזקאל מד:כז) בק''ק (קהל קדוש) ישן יע''א (יכוננה עליון אמן)/ ביום ראש השנה/ ש' (שנת) קעב ליצירה."
"El Shadai (God Almighty), this ornament and the mappah (cloth) were dedicated by Rabbi Moses Joseph, May his Rock sustain and protect him, and by his wife Ms. Esther, May she will be blessed above women in the tent (Judg. 5:24), for the miracle that the blessed Lord made and saved them and their house from fire. 'Blessed art Thou who redeems and save.' And when it (the plaque) goeth into the Sanctuary (based on Ex. 28:29; Lev. 10:18, 16:23; Ezek. 44:27) (donated) in the Old Holy Congregation, may the Sublime establish it, amen, on the day of Rosh Ha-Shanah (the New Year), the year 172 ((5)542) 19.09.1811 to the Creation (see: Remarks: no. 2)."
A suspension ring is inserted through a loop attached to the top of the plaque.
