The monument is located at the killing site in the Gluboke Tract. It is a white stone slab with finished surface on its front side. The left surface is upright, while the right side surface is chipped. The monument bears Hebrew, English, and Ukrainian inscriptions beneath a Magen David.

Translation: To the memory of the Jews, who were murdered by the German Nazis in this field in Mykulychyn in 1941–1942. May their memory be blessed.

אויף אייביק לזכרון עולם פאן אונדזערע יידישע פאמיליעס וועלכע זענען
געהארגעט געווארען דורך די דײתשן און זײערע מיסהעלפער אויף דעם

According to Liubov Solovka, there were 350 Jews in Mykulychyn in 1939. All of them were murdered together with some Hungarian Jews in the Gluboke Forest near Mykulychyn in October 1941 [Solovka, p. 151].
According to the Tsal Kaplun Foundation, Ruth Rosenstock-Michel, the only Holocaust Survivor from Mykulychyn and a daughter of the Holocaust Victim in Mykulychyn Aaron Rosenstock, erected a memorial plaque on the killing site.
A new monument on the mass killing site in Mykulychyn was erected in 2020. A member of Nadworna Shtetl Research Group NGO Darcy Stamler, inspired after the installation of the monument in Nadvirna in 2018, engaged Ms. Yavirnytska to install the monument [Rohatyn Jewish Heritage].
