Description:
The monument is located in the Southeastern Alley of the cemetery, along with dozens of other Holocaust memorials built by Landsmannschaft organizations, survivors, and relatives. It is a light-colored stone slab standing on a matching light stone base. On the base lies a small, flat stone slab. The monument is enclosed by a metal fence. The monument bears Hebrew inscriptions honoring the community’s memory. The community names are emphasized through the use of a larger font size.

In memory/of the martyrs of the communities of/Grodzisk/
and Błonie/near Warsaw/who perished in the Holocaust/5699–5705 [1939–1945]/May God avenge their blood.

As seen on the Ghetto Fighters' House Archive website, originally, at the front of the monument there was a small stone box with a transparent door intended for memorial candles. To the right, there was a stone urn. The base itself was covered with dark stones.
