The oval dedicatory plaque bears a dedicatory inscription in its center.

The inscription is written in square linear letters and reads: "Dedicated to the Lord / this  plaque is a donation of  Tzalha / daughter of Frida for the exaltation / of the soul of Eliyahu son of / Simcha / may his soul be bound up in the bond of life"

Two flowering branches rise from the bottom of the plaque, forming a rhomboid frame around the inscription.
