Description
The monument is located at the site of the killings in a forest near Yelykhovychi. It consists of several stone slabs with inscriptions. Three upright rectangular slabs stand close together at the front: the left slab bears a Hebrew inscription, the central one has a Ukrainian inscription, and the right slab features an English inscription. All three slabs are adorned with a Star of David at the top and a Menorah at the bottom. The central slab also includes the Ukrainian coat of arms at the top, alongside the Star of David. In front of the three slabs lies a rectangular stone featuring an English inscription listing the names of the victims. On either side of the three front slabs, three additional slabs extend outward, forming two long panels facing each other, each inscribed with the names of the victims. The area within this complex of slabs is paved.

Translation: At this site are located mass graves of thousands Jews from Zolochov and his vicinity, that were murdered by the Nazis and their assistants in 1943 – 1944 / Families of the Victims are dedicated this tombstone to the memory of all their loved ones, may God avenge their blood, that burial place of many of them is not known. We will never forget. 5768 – 2008.

In April 1943, during the liquidation of the Zolochiv ghetto, approximately 3,500 Jews were murdered in a forest in Yelykhovychi [Tsal Kaplun Foundation].
In 2008, the families of Jewish Holocaust Survivors from Israel and other countries erected the monument in Yelykhovychi, on the initiative of Roald Hoffmann, one of the Survivors and a native of Zolochiv [Zaxid.net, Yad Vashem; the date is based on the inscription on the monument].
On July 15, 2009, the unveiling ceremony took place [Zaxid.net].
