Description:
The monument is located in the Eastern Alley of the cemetery, along with dozens of other Holocaust memorials built by Landsmannschaft organizations, survivors, and relatives.
The monument is a tall rectangular light stone slab. To its right stands a black stone panel with dozens of small golden plaques listing names. The monument stands on a dark stone base. There is a metal plaque and niche on the reverse side of the stone monument.The monument bears Hebrew inscriptions honoring the community's memory. The community name is emphasized through the use of a larger font size.

on the front of the tall stone monument, in Hebrew:
Translation: Sieradz community / Memorial day: 6 Elul / Annihilated in the Holocaust 5702 [1942]
on the black panel, in Hebrew:
Translation: We will remember and not forget / Let us remember the martyrs of our town Sieradz / whose entire families perished in the Holocaust / and whose names are not listed here [list of victims]
on the reverse side plaque, in Hebrew:
Translation: Sieradz community.
