It features an image of the synagogue and identical inscriptions in Croatian, Hebrew, and English. The inscriptions do not mention the Holocaust.

The plaque was unveiled on September 13, 2021, in the presence of the Israeli Ambassador to Croatia, the chief rabbi Moše Prelević, the head of the Zagreb Jewish community Ognjen Kraus, the mayor of Karlovac Damir Mandić, the County Prefect Martina Furdek Hajdin, the founder of the association “Jews in Karlovac,” Tena Bunčić, and others.
