Description
The monument is located on the site of the former ghetto. The pyramid-shaped monument sits atop a square base and consists of four parts. The upper part has a round top and bears a Magen David. Each subsequent part bears Hebrew, Ukrainian, and English inscriptions respectively. The back side of the monument has a few plaques with Hebrew and English inscriptions.
The area of the monument is paved and fenced, it has a metal double-sashed gate with an arch. There is a Magen David at its top and two Magen Davids on each sash.

Translation: In memory of three thousand Jews, members of the Tuchyn community and surroundings, who rebelled and fought against Nazi murderers and their criminal assistants / burnt the ghetto houses / were burnt alive while trying to escape, shot, murdered in the fields, and towns in September 1942. // Respect to their holy memory

В памʼять про три тисячі євреїв
мешканців Тучина котрі ціною
неймовірних зусиль здійснили
повстання в "Гетто Тучин". Були вбиті,
спалені та поховані заживо нацистами -
вбивцями та їх прибічниками -
злочинцями в вересні 1942 року.
Світла їм памʼять!
Translation: In memory of three thousand Jews, residents of Tuchyn, who, at the cost of incredible efforts, carried out uprising in the "Tuchyn Ghetto." [They] were killed, burned out, and buried alive by the Nazis – murderers and their supporters – perpetrators in September 1942. Blessed memory to them!

The German Army entered Tuchyn in July 1941. A ghetto in Tuchyn was established in Spring 1942. During the liquidation of the ghetto on September 23–24, an uprising started, which lasted until September 26 of that year. About 2,000 Jews were killed in Tuchyn [Tsal Kaplun Foundation].
Zeev Portnoy, Yosef Zilberberg, and Miriam Schwartzman Koutz’s son Gideon erected the monument in 2011 ["Conclusion"].
