The head plate is flat and consists of two concentric circles, one of ivory, the other of wood. The inner ivory plaque has a hole in its center for the stave, surrounded by an incised double circle. Small circles, followed by another double circle, frame it. Around the ivory plaque, in the wood, are two inlaid linear circles, between which are inlaid fifteen mother-of-pearl circles. An inscription is engraved on the ivory plaque and appears around its outer edge:
"ז'(זכר) פריידא בת מו'ה (מורנו הרב) דוד טעביל שנת ת'ר'ע'ד"
"In the memory of Freida, daughter of our teacher, the Rabbi David Tebil, the year (5)674 (1912)"
