The medal bears a depiction of two gothic arches in the center. In one arch there is a depiction of a Jew in a round hat and a beard sowing the land and in the other there is a depiction of people at the Western wall. In the center between the arches there is a Star of David. There is an inscription, in linear letters, flanking the arches which reads: "אם אשבחך ירושלים תשכח ימיני )תהילים 731:5 (". "If I forget thee O Jerusalem May my right hand forget her cunning (Psalms 137;5)".
The revers side presents a landscape with the settlement Rishon LeZion surrounded by fields, with the date: "תרמ"ב" "642=(1882)". Around the depiction is an additional inscription which reads: "ראשון לציון הנה הנם ולירושלים מבשר אתן )ישע' 14: 72 (" "The first shall say to Zion, Behold, behold them and I will give to Jerusalem one that bringeth good tidings (Is.41;27)".
There is a small ring on top of the medallion for a chain.
